  2. Create an account: Sign up with your email address to access the self-exclusion form.
  3. Complete the form: Answer a series of questions to provide your personal and identifying information.
  4. Upload documentation: You will need to upload the following photos:
    • A profile photo of yourself.
    • A profile photo of yourself holding your identification.
    • A photo of the front and back of your identification.
  5. Select an exclusion period: The minimum exclusion period is three years. You can also select a period of up to ten years. A lifetime exclusion is available if you have already completed a minimum three-year exclusion.
  6. Digital signature: Sign the form digitally. After submitting it, you will receive an email confirmation.

The entire process is designed to be user-friendly and secure, ensuring that your information is handled confidentially. Once your exclusion is processed, you are prohibited from entering gaming areas at all participating facilities for the duration you selected.
